Dim Light Challenge: Symfony vs. Vivity IOLs in Cataract Surgery—Visual Outcomes Revealed
The clinical trial titled "Evaluation of the Symfony With Optiblue Intraocular Lens Compared to The Vivity IOL Under Mesopic Conditions" (NCT06229106) investigated visual performance of two distinct intraocular lenses in adult patients undergoing cataract surgery. Conducted by Carolina Eyecare Physicians, LLC with collaboration from Science in Vision, the study specifically measured intermediate and low-contrast visual acuity under mesopic (dim light) conditions—a scenario critical for many daily activities.
Study Design and Population
This Phase 4, randomized, double-masked, parallel-assignment trial enrolled 64 participants aged 50 years or older who underwent bilateral lens extraction targeting a plano refraction. Participants were randomized to receive either the Symfony Optiblue IOL (n=32) or the Clareon Vivity IOL (n=32). Surgeries were performed at two US sites: Center for Sight (Florida) and Carolina Eyecare Physicians (South Carolina). Thirty-one subjects completed the study in each arm.
Study Methodology
The trial applied double-masking (participant and outcomes assessor) and required subjects to read a visual acuity chart at different distances under mesopic lighting at 3 months post-surgery. The primary outcome was binocular distance-corrected intermediate visual acuity at 66 cm (logMAR). Secondary endpoints included acuity at 40 cm and low contrast visual acuity at 4 meters.
Cohort Characteristics
Both groups had similar demographic distributions. The mean age was 70.17 years (SD 6.31), with 30 females and 34 males across the full cohort. The racial makeup was predominantly White (94%), with a small representation of Black or African American participants (3%) and one participant identifying with more than one race. Most participants were not Hispanic or Latino.
Visual Outcomes at 3 Months
For the primary endpoint at 66 cm, the mean logMAR visual acuity was 0.27 (SD 0.17) for Symfony and 0.32 (SD 0.11) for Vivity. At 40 cm, mean logMAR acuity was 0.54 (SD 0.15) for Symfony versus 0.52 (SD 0.12) for Vivity. For low contrast acuity at 4 meters, mean logMAR was 0.44 (SD 0.1) for Symfony and 0.43 (SD 0.1) for Vivity. Each arm had 31 evaluable participants for these outcomes.
Safety Profile
No deaths or serious adverse events were reported in either group during the 3-month follow-up period, and there were no other adverse events above the reporting threshold.
Reflecting on the Findings
This study provides comparative data on two extended-depth-of-focus intraocular lenses under mesopic conditions—a clinically meaningful scenario for cataract patients. The results indicate that both lenses yield similar low contrast and near/intermediate vision performance in dim light.
